*DECLARATION* I hereby declare as below: **I believe in and am fully committed to the promotion and protection of human rights in accordance with the Universal Declaration of Human Rights and all other United Nations Charters, Covenants and protocols on human rights. **I believe in the right of everyone, individually and in association with others, to the realization and enjoyment of all human rights and fundamental freedoms regardless of their gender, religion, caste, creed, ethnicity, domicile, political affiliation or social status. **I fully agree with the Action for Humanity Mission and commit to the promotion and protection of these values and principles to the best of my ability, and pledge to remain engaged with Action for Humanity activities for the achievement of its aims and objects.

PLEASE NOTE **Applicant Has a record of engagement with issues, causes or movements for the promotion and protection of human rights, democracy, rule of law and social justice at the local, regional or national and international levels. **Has not engaged in any serious human rights violation, nor has by words spoken or written expressed opinions or demonstrated a conduct repugnant to the values and principles enshrined in international human rights instruments, the fundamental rights guaranteed in the Constitution of Pakistan and the Action for Humanity aims and objects. **Pledges to remain engaged with Action for Humanity activities to a reasonable extent of her/his availability. **Submits her/his contact mailing address, mobile telephone and WhatsApp numbers on SIM connections registered with PTA and/or email address, verified by the Senior Citizen of Area or Chairman of the Union Council along with 2 Witnesses, a copy of her/his CNIC, and a recent photograph, verified by the Senior Citizen of Area or Chairman of the Union Council along with 2 Witnesses, with the application.
